Come up with creative and actionable New Year's resolutions that will help you manage your diabetes and your overall health.

In this 2 part series, we help you prioritise the resolutions that will make the most significant difference in your health and well-being.

1. See your doctor 2-4 times a year.

Most doctors are happy to see you every three months for blood tests, including the A1C test, but if diabetes is under control and you're not taking insulin, you may only want to see a doctor every month.

Schedule an eye exam at least once a year, a podiatrist, and one dental exam every six months.

2. Keep track of your numbers

Keep track of all your numbers to track your diabetes control and other health conditions. Find out how high your blood pressure, cholesterol (especially LDL or bad cholesterol) and diabetes A1C levels are. Always have enough blood glucose test supplies on hand, and make sure they are not expired. Resolve to record numbers during home testing using a logbook or computer spreadsheet.

3. Develop and Stick to a Good Meal Plan

Establish a balanced diet high in non-starchy vegetables, lean meats, poultry, fish, low-fat dairy and whole grains. Avoid regular sodas and fruit juices, even if they claim to be fresh and natural. Limit alcoholic beverages to one drink per day for women and men over the age of 65 and two drinks per day for men under the age of 65. Add zero-calorie teas and coffees for extra flavour and health benefits.
